Parent Acknowledgements

I understand that these are on-water fishing excursions aboard a charter vessel.
I understand that all participants must wear a USCG-approved life jacket while on board.
I have disclosed all allergies, medications, dietary restrictions, and medical information relevant to safe participation.
I understand that weather, sea state, mechanical issues, or captain judgment may change the fishing plan, route, timing, or date.
I authorize Bloodline Charters LLC to seek emergency medical care for my child if I cannot be reached promptly.
I understand that parents should pack personal snacks or lunch for dietary needs, even when food is provided on board.

Payment & Cancellation Terms

The excursion price is $750 per participant. Full payment is due at signup to reserve the spot.

Cancellations must be made by phone or email at least 4 weeks prior to the start date to receive a full refund.

The captain has the right to cancel or stop any session at any time due to weather or safety conditions. Although the crew may go out in rain, sessions stay ashore in hazardous conditions, including wind, lightning, heavy rain, or other unsafe conditions.If a session day is postponed due to weather, Bloodline will schedule a make-up day. Refunds are unavailable when a participant cannot attend the scheduled make-up day.

Release & Assumption Of Risk

I understand that boating and fishing involve inherent risks, including changing weather, vessel motion, fishing hooks and equipment, marine conditions, sun exposure, and other hazards associated with time on the water.

To the fullest extent allowed by Massachusetts law, I acknowledge these risks on behalf of myself and my child, and I agree that Bloodline Charters LLC may rely on the information and authorizations provided in this packet.
